Only one has been seen, but on multiple occasions, those occasions all share the background of cheating in a nuzlocke challenge. it is said to come to those who have cheated against the rules of a nuzlocke and comes to reap the pokemon that shouldve died.

It shows no fear nor any emotion to anyone whether it be the ones it reaps or innocent bystanders.

It has no intrest in other people besides the one its hunting.

Its ghost tail, axe end is the heaviest part of its body weighing at about 200 pounds.

Its center mouth seems to have a sentient mind of its own, with its eye inside the mouth acting as the controler of the arms, and the top head having no use but to strike fear in its enemies.
